I, the undersigned, hereby authorize MyRxWallet North America Corporation ("MyRxWallet") to act as my personal health record representative and to request, receive, and store copies of my Protected Health Information (PHI) from any and all healthcare providers, hospitals, clinics, laboratories, pharmacies, insurance carriers, and other covered entities that hold records pertaining to my medical care.

Purpose: To consolidate my health records into my personal MyRxWallet vault, under my exclusive control, enabling me to manage, share, and monetize my own health data in accordance with HIPAA and applicable state law.

Information to be disclosed: All medical records including but not limited to: clinical notes, diagnoses, laboratory results, imaging, prescriptions, surgical history, immunization records, and insurance claims.

Expiration: This authorization remains valid until revoked by me in writing via my MyRxWallet account settings.
